I have worked in software distribution and this is one of the most common places that effective and wide ranging viruses get introduced. I had an employee who decided to do this to the distribution for their own profit. Sometimes it can be done by a competitor. It requires constant vigilance by the manufacturers and attention to their own houses. In my case the only cost was firing an employee, rewriting all the disks, and a delay. I personally checked every product myself for this very reason.
